Male Breast Reduction Surgery/Gynaecomastia

Male breast reduction surgery can be life-changing for those that have it. Gynaecomastia is a common issue among men, yet it is rarely discussed. The male breast is made of both fat and breast glandular tissue. While some young men experience a small lump of breast gland behind the nipple during puberty, which typically disappears, many men are left with a small breast on one or both sides. Sometimes men can experience an increase in breast size due to age-related weight gain, which also results in gynaecomastia. It can cause great embarrassment and lead to avoiding activities like swimming.

The treatment required depends on various factors, such as the amount of fat, the size of the gland, and the tightness or looseness of the skin. Male breast reduction surgery involves excision of the gland and possibly a small amount of liposuction. It is a priority to try to minimise scarring and hide any scars as discreetly as possible. If the skin is tight over the gynecomastia, liposuction may be all that’s required. However, if the gland has also enlarged, an incision will also be needed as the gland is often too fibrous to be removed via the liposucker. Patient 1

This gentleman has bilateral glandular gynecomastia. He has quite tender glands underneath the nipple-areola complex. The nipple-areola complex has prolapsed a little bit, creating a puffy look. Surgery cannot be performed with liposuction alone and so Graham had to remove some skin around the areola each side at the same time as removing the gland.

Patient 2

This patient had liposuction and an excision of the small gland, this is more obvious on the left side.

Patient 3

This patient underwent liposuction as well as small gland excision. The area can become quite bruised and lumpy for several weeks postoperatively.

Patient 4

This shows a form of Gynaecomastia which made the nipple/areolar complex “pointy”. A resonable correction has been achieved through a periareolar incision and Bodyjet liposuction.

Patient 5

The skin here is looser. A periareolar excision has tightened this sufficiently. Sometimes the skin is too loose to be tightened in this way and requires another technique.

Patient 6

Glandular excision through an incision around the areola. This was combined with Bodyjet liposuction.
